How Can We Help You?

When a client makes an enquiry we take contact details, purpose of the contact and the needs to be fulfilled.

We then arrange to complete a full assessment to be completed at the client’s place of residence or hospital. Following the assessment a care plan will be devised which our professional care team will follow.

During this process we will also provide information and support with the following, if applicable:

  • Respite hours for family caregivers
  • Power of Attorney assignment
  • Estate Planning
  • Benefits: We can help signpost you to for appropriate financial assessments & support with completing application forms
  • Senior Social Events
  • Blue Badge Application
  • Community Based Programs
  • Transport Services
  • Coordinating Doctor’s Appointments
We can provide assistance to discuss, plan & coordinate your care plan & provide legal guidance if required
Use our transport service to get to your appointments, shopping or leisure activities safely

Navigating through the myriad of care services available in the community can often be cumbersome and confusing. For help in identifying the best care service options for you or your loved one, Clifton Homecare can help ease the burden.

We will work with the attending Physician, Discharge Planner, family and the client to determine the care plan and agree on the specific goals to maximise the healing process.

We can provide assistance with recommending, coordinating & referring other services to aid your mobility & advise on fall prevention

Clifton Homecare will also assist in coordinating and referring other services needed such as:
